目的：为临床合理使用糖皮质激素提供参考。方法：抽取某院2013 年1 月－2015 年6 月门诊各科室使用糖皮质激素的处

方，对糖皮质激素使用情况、处方科室分布、药品使用频次、处方诊断分布等进行统计分析。结果：15 000 张处方中，使用糖皮质激

素的有1 562 张，使用率为10.4％；其中判断为不合理的处方有189 张，不合理率为12.1％。糖皮质激素使用比例最高的科室为耳

鼻喉科（27.66％），不合理用药比例较高的是儿科（16.12％）；使用频率最高的是地塞米松注射液（44.88％）；使用糖皮质激素处方

诊断最多的是急性支气管炎、支气管炎（267 张处方）。结论：该院在糖皮质激素使用过程中存在不合理用药现象，应引起重视并

进一步加强处方审核和点评，以确保糖皮质激素在临床使用的安全、有效。

OBJECTIVE：To provide reference for rational use of glucocorticoid in the clinic. METHODS：The glucocorticoid

outpatient prescriptions collected from clinical departments of a hospital during Jan. 2013 to Jun. 2015 were analyzed statistically

in respects of glucocorticoid use，department distribution，DDDs，distribution of prescription diagnosis，etc. RESULTS：Of

15 000 prescriptions，there were 1 562 glucocorticoid prescriptions，with utilization rate of 10.4％；189 prescriptions were recognized

as irrational ones，with irrational rate of 12.1％. The proportion of glucocorticoid in otorhinolaryngology department was

the highest（27.66％），and irrational drug use was the highest in pediatrics department（16.12％）；Dexamethasone injection was

the most widely applied（44.88％）；most of glucocorticoid prescriptions were used for acute bronchitis and bronchitis（267 prescriptions）.

CONCLUSIONS：The irrational glucocorticoid application has existed in the hospital，which deserved special attention.

Hence，prescription check and evaluation should be strengthened to promote the safety and efficacy of glucocorticoid in clinical

application.
