摘要: | 目的:分析北京医院(以下简称“我院”)地高辛血药浓度监测结果及其影响因素,为临床合理用药提供参考。方法:选择我院1996-2015年监测地高辛血药浓度的患者3 385例,采用荧光偏振免疫法测定其血药浓度,分析患者性别、年龄、给药剂量等因素对血药浓度的影响。结果:我院地高辛用药监测率从2002年的78.9%下降至2015年的37.2%,呈下降趋势。3 385次监测结果显示,地高辛平均血药浓度为(1.05±0.04)ng/ml;血药浓度在有效治疗范围(0.5~1.75 ng/ml)内的有2 391例(占70.64%);女性患者平均血药浓度[(1.06±0.04)ng/ml]略高于男性患者[(1.05±0.04)ng/ml],但差异无统计学意义(P>0.05);患者年龄以≥70岁为主,男、女性患者年龄均与血药浓度呈正相关[相关系数(r)分别为0.923、0.958,P均为0.000 2];患者给药剂量与血药浓度不相关(r=0.773,P=0.125)。结论:我院地高辛的使用日趋合理,其血药浓度主要受患者年龄的影响。临床应结合血药浓度监测结果及患者实际的临床症状,制订合理的个体化给药方案。 |
ABSTRACT: | OBJECTIVE: To analyze blood concentration monitoring data of digoxin in Beijing Hospital (called “our hospital” for short) and its influential factors, and to provide reference for rational use of drugs in the clinic. METHODS: 3 385 patients underwent blood concentration monitoring of digoxin were selected from our hospital during 1996-2015. Blood concentration of digoxin was detected by fluorescence polarization immunoassay. The effects of patients’ gender and age, drug dose and other factors on blood concentration were analyzed. RESULTS: The rate of digoxin medication monitoring in our hospital decreased from 78.9% in 2002 to 37.2% in 2015, showing a descending tendency. Results of 3 385 times monitoring showed that average blood concentration of digoxin was (1.05±0.04)ng/ml; blood concentration of 2 391 cases were in normal range (0.5-1.75 ng/ml), accounting for 70.64%. Average blood concentration of digoxin in female patients [(1.06±0.04) ng/ml] was slightly higher than in male patients [(1.05±0.04) ng/ml], without statistical significance (P>0.05). Most of patients were aged ≥70 years, and patients’ age was positively correlated to blood concentration [relative coefficient (r) were 0.923 and 0.958, both P=0.000 2]; drug dose was not correlated with blood concentration (r=0.773, P=0.125). CONCLUSIONS: The application of digoxin become more and more reasonable in our hospital, and its blood concentration is influenced by patients’ age. Individual therapy regimens should be formulated according to blood concentration monitoring and clinical symptom of patients. |
